Screening

Our screening program is free of charge. It involves our staff, including Dr. Metzger, coming out to your facility. We will bring everything that is needed. It is normally done at team practices, and is done in a way that it does not interfere with practice. It has been done at league opening day ceremonies, with as many as 200 players screened in about a 3 hour period! Each screening exam takes a little more than one minute.

If you print the History Form and the Study Consent Forms below, fill them out and bring them to your screening, it will make it even faster! Please note that parents must sign the consent for all minors under the age of 18.

Letter to League Officials
A description of our program for league officials such as regional or district administrators, league presidents, or team managers and coaches.

Letter to Parents of Youth Throwers
A description of our program geared toward the parents of youth throwing athletes.

Study Consent Form
Print this form and bring it to your child's screening. Forms will be available at the screening, but must be signed by a parent or legal guardian.

Throwing History Form
The 1-page form each athlete will complete at the time of screening, so SafeThrow staff can understand any pertinent information about the throwers' arm. Feel free to print it, and complete it before your screening! Then screening will take only about 30 seconds!

Throwing Medical Form
This should be completed before screening only if the athlete has any history of problems with the throwing arm, and would like to discuss those with Dr. Metzger.
